There are two plans available for
Seniors: the PPO plan and the Basic PPO plan. Please
note that we no longer offer an HMO plan for Subscribers
over the age of 65 year. Please have the Subscriber
call our office for details. |

age 65 soon, what should I do? |

It is
important to have the Subscriber call our office three
(3) months prior to them turning age 65, so that
coordination with Blue Shield of California and Medicare
will be trouble free. |

Why doesn’t
JACL Health Benefits Administrators have a drug card? |

|
|
A: |
JACL
Health Benefits Administrators works very hard to
keep premium costs affordable by working with Blue
Shield in customizing our health plans. Our PPO plans
have no drug card because of the additional overhead
costs associated with a drug card program. This has
allowed us to maintain unlimited prescription drug
coverage.
Please note that our HMO and Active Choice 500 plans
have drug cards. |

What kind
of prescription drug coverage do the JACL Health
Benefits Administrators plans have? |

For
PPO Subscribers, after your calendar year deductible
is met, the plan reimburses you for 80% of prescription
drugs to an unlimited amount. In addition, the plan
does not limit you to generic drugs, rather you can buy
name brand drugs if you so choose. For HMO and
Active Choice Subscribers, the prescription drug
co-payment is only $15.00 per prescription for generic

There
are specific requirements for adding dependents. It is
a good idea to have the Subscriber call our office
immediately if they want to add a dependent to their
plan. |

be done in the event of the death of a Subscriber? |

A: |
It is
important to have a member of the Subscribers family
contact the JACL Health Benefits Administrators office
as soon as possible for two reasons: if the deceased
Subscriber has dependents on his or her policy, they
need to be transferred to their own policy; and a refund
may be due. |
